Internal bleaching is a specialized whitening procedure used to lighten the discoloration of a tooth that has undergone root canal treatment. Unlike external teeth whitening methods, which target surface stains, internal bleaching is performed from within the tooth to address deep discoloration caused by blood breakdown products, necrotic pulp remnants, or medications used during endodontic treatment.

How Does Internal Bleaching Work?

Discoloration of a non-vital tooth often occurs after trauma, pulp necrosis, or previous root canal therapy, where remnants of blood or bacteria stain the dentin. The internal bleaching procedure begins with the placement of a protective barrier over the root canal filling to prevent irritation or leakage.

A bleaching agent, typically sodium perborate mixed with water or hydrogen peroxide, is placed inside the pulp chamber. The access cavity is then temporarily sealed, allowing the bleaching agent to work over a few days. This process may be repeated until the desired shade is achieved, after which the bleaching material is removed, and a permanent restoration is placed to seal the tooth.

Aftercare & Monitoring

Internal bleaching is a safe and effective method for restoring the natural appearance of a discolored tooth. However, it is only suitable for non-vital teeth with intact root canal fillings. In some cases, additional restorative work, such as a veneer or crown, may be needed if the tooth has structural damage.

Proper case selection and careful monitoring are essential to minimize risks such as external cervical resorption, which can occur if the bleaching agent diffuses beyond the intended area. When performed correctly, internal bleaching provides excellent aesthetic results, helping patients regain confidence in their smiles.
